On September 19, 2024, Fee Insurance became aware of suspicious activity related to the email account of one employee. As a result, Fee Insurance launched an investigation to confirm the full nature and scope of the activity.  The investigation determined there was unauthorized access and/or acquisition of certain files and folders from the relevant mailbox between September 16, 2024 and September 20, 2024.  Following this discovery, Fee Insurance began an extensive review of these files and folders to determine what information was present within the impacted files and folders and which individuals might have been impacted.  This review and a subsequent manual process of identifying address information were completed on April 22, 2025.  Please note that we do not have any evidence to indicate that your information was subject to actual or attempted misuse as a result of this incident.

While the affected information varies for each individual it can include: name, Social Security number, driver's license number, health insurance information, and medical information.

Fee Insurance encourages potentially impacted individuals to remain vigilant against incidents of identity theft and fraud, to review account statements, and to monitor their credit reports and explanation of benefits forms for suspicious activity.
